FOR CANADA: The form you need is Form NR7-R, Application for Refund of Part XIII Tax Withheld. You can find it on the Canada Revenue Agency (CRA) website: https://www.canada.ca/en/revenue-agency/services/forms-publications/forms/nr7-r.html
To mail your request, you can use the following address:

Please note that you must include the following documents with your request:
A copy of your Form 1042-S, Foreign Person’s U.S. Source Income Subject to Withholding Tax
A copy of your W-2G, Gambling Winnings (But Not Tips) and Other Income
A detailed statement of your gambling winnings and losses for the year in which you had the withheld tax
Any other supporting documentation, such as receipts or casino statements
You can also submit your request online through the CRA’s Netfile website. However, you will need to have a registered My Account with the CRA.
Once the CRA has processed your request, they will send you a refund check or deposit the refund directly into your bank account. The processing time can vary, but it is typically around 6 to 8 weeks.
