  1. Hello! My name is Connie M*****ak and I’m the Purchasing Coordinator at HealthEquity, Inc. located in Draper, UT.

We have a levy to be paid and in order for me to set you up in our accounting system, I will need a current W9 from your office. Would you be able to email me your W9?

Once received, I will release payment.

Thank you in advance for your assistance!
    Connie
