To make an appointment to pick up your MA 1099-G for PUA benefits 2021, you can call the Department of Unemployment Assistance (DUA) at 617-626-5647. The DUA is responsible for issuing 1099 forms to people who received unemployment benefits in Massachusetts.
When you call the DUA, be prepared to provide your Social Security number and date of birth. You will also need to let them know that you are requesting a 1099-G form for PUA benefits 2021.
